Tiffany Smith, PA-C

Tiffany Smith studied pre-medicine at Fairmont State University before graduating Magna Cum Laude from Mountain State University in Beckley, West Virginia with a Master of Science in Physician Assistant Studies in 2007. For the past sixteen years she has provided high quality healthcare to the greater Louisville area, which she now proudly considers home.
With experience in primary care, urgent care and integrative treatments, she has the unique and extensive experience that places her in a position to bring this distinctive health and wellness approach to Louisville. Frustrated by the current healthcare system, she brought together a team of compassionate, dedicated providers focused on identifying and addressing the cause of illness rather than merely treating the physical symptoms. The integrative approach allows her to continue her passion of delivering high quality healthcare to our community without regulatory interruption.
In her spare time, she enjoys sports, gardening, travel and spending time with family and friends.
Jennifer Gondoly, APRN

Jennifer Gondoly, APRN, MSN, FNP-BC, is a board certified Family Nurse Practitioner through the American Nurses Credentialing Center. She grew up in Michigan and graduated from Michigan State University with her Bachelors degree in Human Biology. She also earned her Bachelors of Nursing degree from Lourdes College, now Lourdes University, in Sylvania, OH. She then relocated to New Jersey where she obtained her Masters degree from Rutgers, The State University of New Jersey in Newark, NJ. After living in New Jersey for 5 years, her and her husband decided they were ready to start a family and relocated to Louisville, KY to be closer to his company.
Prior to starting her career in 2019 as a Nurse Practitioner she practiced as a nurse for over 15 years in the acute care setting; in areas such as Medical/Surgical, Telemetry, Orthopedics, and Rehabilitation. She has worked in several hospital systems across the country including the University of Louisville Hospital before obtaining the opportunity to practice in an Endocrinology Clinic.
Her experience in Endocrinology and Diabetes care afforded her the opportunity to be a part of the Radiant team! She enjoys working with diabetics, thyroid disease, hormone management, weight loss management and aesthetics.
Her favorite hobbies include spending time with family and friends, traveling and CrossFit!
Taylor Snyders, PA-C
"I am passionate about empowering my patients to take control of their health. I believe in helping people understand their body and the "why" behind their problems so that we can work together towards a solution."
Taylor brings 10 years of primary and urgent care experience along with a lifelong passion for nutrition. She holds a bachelor's degree in the science of human nutrition, foods, and exercise from Virginia Tech and a master's degree in Physician Assistant studies from the University of Kentucky. She was born near Pittsburgh, PA, grew up in Northern Virginia, and came to call Louisville, KY home. Taylor has a passion for learning about the human body. She feels that understanding the science behind the patient's symptom will help identify and address the root cause.
Taylor lives with her husband, two sons, and goldendoodle, Sydney, near Louisville, KY. She enjoys travel, hiking, yoga, and reading. She is excited to use her experience and knowledge to help patients on their journey to optimal health.
Samantha Straight PA-C
Samantha has 10 years of experience as a dedicated physician assistant in primary and urgent care. She grew up in southern Indiana and graudated Magna Cum Laude from the University of Louisville with a Bachelors of Science in exercise science. She also holds a bachelors degree in Health Sciences and a Masters of Physician Assistant Studies degree from Butler University in Indianapolis, IN.
Samantha strives to empower patients to better understand their health by integrating evidence-based practices, natural remedies and lifestyle modifications. Samantha is excited to help patients achieve optimal health and vitality.
Samantha lives with her husband, two children, and a beloved boxer in southern Indiana. She enjoys spending time with family and friends, traveling, boating and yoga.
Avian Schafer APRN

I strongly believe in lifestyle modification and establishing healthy habits that are sustainable. Although I like the natural approach, I am adamant about working alongside my patients, giving you the freedom to choose options that fit your lifestyle. You know your body best and I am here to work with you.”
Avian Schafer, MSN, APRN, FNP-C, is a board-certified Family Nurse Practitioner through the American Association of Nurse Practitioners. She is native to Louisville; GO CARDS! She
graduated from University of Louisville with her Bachelors in nursing and Bellarmine for Masters degree in Family medicine.
She has experience in primary care with a specialty in functional medicine, pediatrics, as well as aesthetics. Avian lives with her husband, two children-Wyatt and Emmy, dogs, chickens, rabbits, and bees. She loves the Lord and enjoys backpacking, gardening, being a mom.
Cristal Pollak APRN
Cristal Pollak, APRN, MSN, FNP-BC is a National Board-Certified Family Nurse Practitioner (NP). She graduated from Frontier Nursing University with a master’s science in nursing in 2013. During her 10
years as an NP, her practice has included Cardiology, Endocrinology, and Family. Prior to becoming an NP, she graduated Phi Theta Kappa from Saint Catharine College with an associate degree in nursing.
She worked in the Emergency Department and Interventional Cardiology as a Registered Nurse (RN) for 10 years. With over 20 years in healthcare, Cristal is excited to incorporate integrative medicine and aesthetics into her practice. “I think integrative medicine is the best approach to treatment because it treats the
whole body with a functional and lifestyle approach. “She grew up in Kentucky and currently resides here with her husband, 3 children, and 2 dogs. She enjoys amusement parks, traveling, swimming, and hanging out with her family and friends.
Laura Davis APRN

Laura Davis, MSN, APRN, FNP-C, is a board-certified Family Nurse Practitioner through the American Academy of Nurse Practitioners. She grew up in the Northern Kentucky area near Cincinnati and has spent her life going between NKY and Louisville. Laura graduated from the University of Louisville with her first bachelor’s degree in public health education. She then earned both her Bachelor of Science in Nursing and Master of Science in Nursing degrees from Bellarmine University.
Prior to starting her career as an advanced practice provider, she worked as a registered nurse for over 5 years in the acute care setting, mainly working in the Neonatal Intensive Care Unit at Norton Children’s Hospital. She also spent 3 years assisting in the Pediatric Intensive Care Unit and Pediatric Cardiac Intensive Care Unit as an ECMO technician and dialysis registered nurse.
When she began her nurse practitioner career in 2021 by working in the plastic surgery industry, Laura instantly found her love for facial aesthetics and skin health! Since then, she has attended numerous trainings, conferences, and seminars to improve her skills as an injector and provide the highest quality, safest results for her clients. Her experience afforded her the opportunity to become a part of the Radiant Integrative Health team as our Aesthetic Nurse Practitioner.
Laura is excited to over a wide range of anti-aging and skin health services in our office including neuromodulators, dermal fillers, chemical peels, a variety of laser treatments, micro-needling, skin care advice and so much more! She will also be available for IV therapy treatments and acute care visits.
Her favorite hobbies include spending time with all her fur babies (dogs) and family, travelling, riding her Peloton, and going to Cincinnati Bengals games (WHO DEY!).
